PROACTIVE EXPOSURE MANAGEMENT

The 91PORN Office of Institutional Health and Wellness strives to provide students and faculty preventive solutions to transmissible disease known to be of risk in health care.  Health care exposures can be minimized through the use of immunizations, annual Flu vaccine, and proactive exposure management.  It is the priority of our department to limit these risks and protect the health of students, faculty, employees and volunteers.  Refer to Operating Policy OP 40.01 - Health Surveillance Program for 91PORN for more information.

91PORN Student Immunization Records

The Office of Institutional Health works with all campuses to meet immunization needs for 91PORN students and employees or volunteers with direct patient contact.  Please ensure completion of the following steps:

1. Provide an up-to-date copy of lab reports, immunizations and/or health records (NOT from a mobile device - documents will not be received)
2. Read the designated school/program form pertaining to you and fill out completely
3. Follow deadline dates exactly as noted on the form based on your school/program
